Please fill in this form as completely as possible to expdite
your order. It covers all the major factors involved in
defining an icon project. Please use the Notes field
whenever you select from a drop-down box an option
that ends in ... (for example: Language: Other...)
For murals there will almost certainly
have to be telephone conversations and perhaps a preliminary
site visit to take measurements.
Upon receipt of your request, I will prepare a standard
Contract. Based on your input below, it will specify
the price and the earliest possible delivery date.
If you request ground mail service, I will send you two (2)
copies; if you request e-mail contact, print two (2) copies
on receipt. In either case, please sign both and return one
to me by mail, within thirty (30) days, along with any
special instructions not included below, and
a 50% deposit. The balance, plus any shipping costs,
will be due on delivery.
